Sudden Cardiac Arrest: Be Prepared When Seconds Count

DEFIB_PADSOne of the leading causes of the death in the United States is sudden cardiac arrest (SCA), striking indiscriminately people of all ages and fitness levels.  In fact, many statistics indicate that SCA is a public health crisis with 13% of all workplace fatalities stemming from SCA and a whopping 365,000 people in North America alone falling victim every year to sudden cardiac arrest.  The definitive treatment for the life-threatening cardiac arrhythmias such as ventricular fibrillation, the most common electrical mechanism in cardiac arrest, and the pulse-less ventricular tachycardia, is defibrillation.  Defibrillation is the delivering of a therapeutic dose of electrical energy through a device called a defibrillator to depolarize a critical mass of the heart muscle, terminating the arrhythmia and allowing normal sinus rhythm to be reestablished by the sinoatrial node.

In the past most external defibrillation shocks were delivered via paddles placed upon the patient’s chest delivering a monophasic waveform; current traveling in one direction through the heart.  Currently in the US, the majority of defibrillation shocks are delivered through defibrillation electrodes or pads that are placed directly on the patient’s skin. These defibrillation pads are safer for first responders because they conform to the chest and are generally able to deliver the current more effectively.  Another change in modern defibrillation is the use of biphasic waveform which is electrical current distributed to the heart in two vectors. The efficacy of the shock is significantly augmented with a biphasic waveform as the peak current required to convert the arrhythmia is reduced and results in less myocardial damage from the shock itself.

The moments before first responders arrive are crucial with even the best EMS departments taking an average of nine minutes to respond. The chances of surviving drop by an alarming 10 percent for every minute that passes between the onsets of SCA and the use of a defibrillator.  According to a study performed by Johns Hopkins University for the Journal of the American College of Cardiology, the application of an automated external defibrillator (AED) in communities is associated with nearly a doubling of survival after the onset of sudden cardiac arrest.
